摘要: 近年来,糖尿病肾病(DN)发病率及进展为终末期肾病的风险均呈上升趋势。既往研究表明细胞衰老在DN中发挥重要作用。纤维化是DN的重要病理结局,研究者将纤维化形成过程中的微环境命名为“纤维化生态位”。细胞衰老相关的基因组不稳定性、表观遗传改变及线粒体功能障碍对于微环境的形成以及疾病的进展具有重要作用。纤维化生态位的主要成分及骨架结构对于促进细胞衰老和疾病进展也具有显著性意义。本文就细胞衰老与纤维化生态位在DN进展的各种联系进行综述,为临床治疗DN及相关药物研发提供新的参考方案。

Abstract: In recent years, the incidence and progression of diabetic nephropathy (DN) to end-stage kidney disease (ESKD) have been on the rise. Previous studies have shown that cellular senescence plays a significant role in DN. Fibrosis is an important pathological outcome of DN, and researchers have named the microenvironment formed during the process of fibrosis as the “fibrotic niche.” Cellular senescence-related genomic instability, epigenetic changes, and mitochondrial dysfunction are crucial for the formation of the microenvironment and the progression of the disease. The main components and scaffold structure of the fibrotic niche also have significant implications for promoting cellular senescence and disease progression. This article reviews the various connections between cellular senescence and the fibrotic niche in promoting the progression of DN, providing new reference schemes for the clinical treatment of DN and the development of related drugs.
