摘要:

目的:研究肥胖相关性肾病(ORG)患者长期预后及相关危险因素。
方法:选取东部战区总医院国家肾脏疾病临床医学研究中心ORG随访登记数据库,分析2003年1月至2015年12月经肾活检确诊为ORG患者的随访资料。利用KaplanMeier法计算肾脏累积生存率,并利用COX回归模型分析预后危险因素。
结果:共227例患者纳入本研究,中位随访时间61月,随访期间37例(163%)新发糖尿病,14例(62%)进展至终末期肾病(ESRD),18例(79%)患者发生终点事件[估算的肾小球滤过率(eGFR)下降50%或进入ESED]。患者肾活检后5年、10年累积肾脏生存率分别为936%和899%。多因素COX回归分析结果表明活检时肾功能不全(HR=6140,P=0001)、重度间质纤维化(HR=9102,P=0045)和时间平均蛋白尿(HR=1578,P=0000)是肾脏预后的独立危险因素。调整活检时肾功能不全、时间平均蛋白尿后,重度间质纤维化较无间质纤维化,终点事件风险增加9102倍(P=0045)。
结论:本研究结果表明ORG患者5年、10年肾脏累积生存率分别为936%和899%。活检时肾功能不全、重度间质纤维化、时间平均蛋白尿是ORG患者预后的独立危险因素。

Abstract:

Objective:To study the prognostic characteristic and related risk factors of patients with obesityrelated glomerulopathy (ORG) in China.
Methodology:Two hundred twenty seven patients with ORG were enrolled in this retrospective study,who were diagnosed by renal biopsy in the Research Institute of Nephrology,Nanjing University School of Medicine from January 2003 to December 2015. We observed the clinical,pathological and prognostic materials of ORG patients and searched related risk factors of  prognosis by Cox regression analysis.
Results:The median of followup in this study was 61 months,the median of ΔBMI was 12 kg/m2.At the last observation,37 patients (163%) suffered diabetes,14 patients (62%) reached endstage renal disease,18 patients (79%) had the end point.The 5 year and 10 year cumulative renal survival rates,calculated by KaplanMeier survival analysis,were 936%, and 899%.Multivariate Cox regression analysis showed that severe renal interstitial fibrosis (HR=9102,P=0045), impaired function of the kidney (HR=6140,P=0001) and timeaverage proteinuria (HR=1578,P=0000) were the independent risk factors.
Conclusion:The 5 year and 10 year cumulative renal survival rates of ORG patients were 936%, and 899%.Severe renal interstitial fibrosis, impaired function of the kidney and timeaverage proteinuria were the independent risk factors for renal function progression in ORG patients.
