Abstract: A 24yearold female as a nondiabetic recipient received renal allograft from a donation after cardiac death (DCD) donor with diabetes of 15 years. After transplantation, mild proteinuria gradual decreased to a normal range after 6 months and the kidney functioned well with standard immunosuppression. Timezero renal biopsy showed evidence of diabetic nephropathy (DN). Sequential protocol biopsies demonstrated gradual resolution of the preexisting DN from 6 months to 4 years after transplantation, with appearance of transplant glomerulopathy. The renal function remained stable at the 6year followup.
