摘要: 目的:探讨维持性血液透析(MHD)患者并发缺铁性贫血的危险因素及列线图模型的建立。
方法:选取2018年1月~2019年5月南方医科大学附属小榄医院肾内科296例MHD患者,调取临床病例信息及一般临床资料,使用Logistic回归分析筛选MHD患者缺铁性贫血的独立危险因素。然后纳入筛选出的独立危险因素建立列线图预测模型,并对模型的预测性及准确度进行验证。
结果:通过对患者基本资料及透析相关资料做logistic回归分析,筛选出高血压、慢性炎症、合并用药情况(rHuEPO、铁剂、叶酸、维生素B12)、铁调素水平、透析参数(透析类型、透析时间及频率)为并发缺铁性贫血的独立危险因素(P<005),均与MHD患者并发缺铁性贫血高度相关,纳入该10项独立危险因素,建立预测维持性血液透析患者并发缺铁性贫血的列线图模型。使用Bootstrap 内部验证法发现该模型具有良好的精准度和区分度,Cindex指数为0791(95%CI 0732~0850)。
结论:通过建立MHD患者并发缺铁性贫血的危险因素分析及列线图模型,能够提高对MHD患者并发缺铁性贫血的预测效能,具有较高的临床应用价值。

Abstract:

Objective:To investigate the risk factors of iron deficiency anemia in maintenance hemodialysis (MHD) patients,and establish a nomogram model.
Methodology:296 MHD patients from our blood purification center from January,2018 to May,2019 were selected to obtain clinical information.Logistic regression analysis was used to screen for iron deficiency anemia.Independent risk factors selected were used to establish a nomogram prediction model,and the predictability and accuracy of the model are verified.
Results:By performing logistic regression analysis,hypertension,drug therapy (rHuEPO,iron,folic acid,vitamin B12),chronic inflammation,hepcidin and hemodialysis parameters were independent risk factors of iron deficiency anemia (P<005).A nomogram model to predict iron deficiency anemia was established and validated,the Cindex was 0791 (95%CI 07320850).
Conclusion:The establishment of risk factor analysis and nomogram model of iron deficiency anemia can improve the diagnostic efficacy of iron deficiency anemia in MHD patients.
