Clinical features and prognosis in patients with hypophosphatemic osteomalacia induced by adefovir dipivoxil

摘要:

目的:分析阿德福韦酯(ADV)导致低磷性骨软化症(HO)的临床特点及预后。
方法:回顾性分析2010年9月至2017年10月在南京总医院国家肾脏疾病临床医学研究中心就诊的12例ADV导致HO的患者,总结该病的临床特点及预后,并与6例未合并HO的ADV相关Fanconi综合征患者对比分析。
结果:12例HO患者,平均年龄481±115岁,服用ADV 47年后出现进行性多发性骨痛,均伴低磷血症、碱性磷酸酶(AKP)升高,75%低尿酸血症、50%低钾血症;肾损害以近端小管功能损害为主,表现为Fanconi综合征[糖尿、氨基酸尿和(或)尿磷排泄增多],伴少到中等量蛋白尿、轻度血清肌酐升高,肾活检病理提示小管间质病变为主;影像学检查提示骨质疏松。12例HO患者与6例未合并HO的ADV相关性Fanconi综合征患者比较,除影像学表现异常、AKP显著升高,其余指标无统计学差异。停用ADV后,骨痛迅速缓解,血磷上升,小管功能亦改善。
结论:长期应用ADV可导致低磷性骨软化症,表现为骨痛、Fanconi综合征,临床上应重视AKP的监测及影像学检查,以免漏诊误诊。

Abstract:

Objective:To analyze the clinical characteristics and treatment approaches of hypophosphatemic osteomalacia(HO) induced by adefovir dipivoxil(ADV).
Methodology:A retrospective analysis was performed according to the clinical data of twelve cases of ADVinduced HO who were admitted in Jinling Hospital, from September 1st, 2010 to October 1st, 2017. The clinical features and outcome were summarized and compared with 6 cases ADVcorrelated Fanconi syndrome without HO.
Results:12 patients were the mean age of 481±115 years, all of them suffered from pain over multiple joints after taking ADV for an average of 47 years, combined the increase of hypophosphatemia and alkaline phosphatase, some of them were accompanied with hypouricemia(75%)and hypokalemia(50%). Main renal damage was proximal tubule dysfunction, with the performance of Fanconi syndromes (glycosuria, aminoaciduria or urinary phosphorus excretion increased), together with less to moderate proteinuria, mildly elevated serum creatinine. Renal biopsy mainly prompted tubulointerstitial lesions; imaging showed osteoporosis. Compared with 6 cases of ADVcorrelated Fanconi syndromes who didnt combine HO, 12 patients suffered HO had abnormal imaging manifestations and obviously higher AKP, other indicators were not statistically significant. After withdrawal of ADV, the bones pain was distinctly relieved, the level of blood phosphorus was increased, and the tubule function were improved.
Conclusion:
Longterm taking ADV could lead to HO, clinical manifestations are bones pain and Fanconi syndromes. We should pay attention to AKP and imaging examination.
