摘要: 包裹性腹膜硬化症(EPS)是腹膜透析(PD)罕见而危重的并发症,主要病理学特征为弥漫性腹膜纤维化并形成致密包膜包裹肠管,进而引发肠梗阻和营养不良。EPS 发病机制尚不明确,目前较为公认的是二次打击学说。常见危险因素包括 PD 疗程延长、腹膜透析相关性腹膜炎反复发作、长期暴露于高渗高糖透析液以及突然停止 PD 等。疾病早期缺乏特异性临床表现,诊断需结合临床症状与影像学评估,其中计算机断层扫描(CT)在识别腹膜增厚、钙化及肠袢包裹方面最具实用价值。治疗需要多学科综合管理,包括停止 PD、强化营养支持、合理应用抗炎及抗纤维化药物,并在指征明确时进行外科手术。本文结合最新研究进展,系统综述 EPS 的流行病学、发病机制、诊断与治疗进展,以期为临床决策提供参考。

Abstract: Encapsulating Peritoneal Sclerosis (EPS) is a rare but serious complication of peritoneal dialysis (PD). Its primary pathological feature is diffuse peritoneal fibrosis with the formation of a dense, constricting fibrous capsule that envelops the intestines, leading to intestinal obstruction and malnutrition. The pathogenesis of EPS remains unclear, with the "two⁃hit hypothesis" being the most widely accepted theory. Common risk factors include prolonged duration of PD, recurrent episodes of PD⁃associated peritonitis, long⁃term exposure to hyperosmotic and high⁃glucose dialysis solutions, and abrupt discontinuation of PD. The early stages of the disease lack specific clinical manifestations, and diagnosis requires a combination of clinical symptoms and imaging evaluation. Among imaging modalities, computed tomography (CT) is particularly valuable for detecting peritoneal thickening, calcification, and encasement of intestinal loops. Management involves a multidisciplinary approach, including cessation of PD, intensified nutritional support, appropriate use of anti⁃inflammatory and anti⁃fibrotic medications, and surgical intervention when clearly indicated. This article comprehensively reviews the epidemiology, pathogenesis, diagnosis, and treatment advances in EPS based on the latest research developments, aiming to offer insights for clinical decision⁃making.
