Impact of inflammation on 6MWT in peritoneal dialysis patients

Abstract:

【Abstract】Objective: To investigate the impact of inflammation on 6-minute walk test (6MWT) of patients with peritoneal dialysis (PD). Methodology: Stable ambulatory patients in Renji Hospital, Shanghai Jiao Tong University School of Medicine between May 1, 2010 and April 30, 2011 were enrolled in present study. All included subjects performed 6MWT, and 6-minute walk distances (6MWDs) were recorded. Patients’ laboratory parameters and echocardiography were also tested. Plasma interleukin-6 (IL-6), and pentraxin 3 (PTX3) were determined by ELISA. Multiple stepwise regression was used to examine the association between inflammatory markers and 6MWD. Results: They were 100 patients, 52 males and 48 females with a mean age of 56.0±12.7 years and had been on PD ranged from 18.8 to72.2 months (a mean duration of 41.1). 17 patients had diabetic comorbidity, and 8 had cardiovascular disease (CVD). 6MWD was negatively correlated with age (r=-0.475, P<0.001), lnNT-proBNP (r=-0.217, P=0.031), lnhs-CRP (r=-0.231, P=0.025), lnPTX3 (r=-0.271, P=0.009), and lnIL-6 (r=-0.270, P=0.008), while it was positively correlated with systolic pressure (r=0.256, P=0.011), diastolic pressure (r=0.370, P<0.001), left ventricular ejection fraction (LVEF) (r=0.261, P=0.013), pre-albumin (r=0.297, P=0.003), and normalized protein catabolic rate (nPCR) (r=0.362, P<0.001). Multiple stepwise regression showed that lnPTX3 was independently associated with 6MWD (β=-0.161, P=0.045), age (β=-0.403, P<0.001), female (β=-0.176, P=0.043), diastolic pressure (β=0.335, P<0.001) and lnNT-proBNP (β=-0.161, P=0.026) were other independently associated factors of 6MWD. Conclusions: PTX3 is significantly associated with 6MWD, the relationship between inflammation and functional exercise capacity of PD patients deserve further study.
