Advances in renal risk score prediction model for antineutrophil cytoplasmic antibody associated glomerulonephritis

摘要: 抗中性粒细胞胞质抗体相关肾炎(AAGN)是由抗中性粒细胞胞质抗体(ANCA)介导的,病理以寡免疫复合物性坏死性新月体肾炎为特征的一组疾病,临床常表现为急进性肾小球肾炎,部分患者可快速进展至终末期肾病。及时、准确识别患者预后危险因素,对于指导临床医生治疗方案的决策、提高患者预后极为重要。最新研究提出AAGN肾脏风险评分(ARRS)模型,通过纳入正常肾小球比例,肾小管萎缩/间质纤维化比例及就诊时估算的肾小球滤过率水平建立了AAGN患者肾脏预后预测模型。本文就该预测模型的研究进展做一综述。

Abstract: Antineutrophil cytoplasmic antibody associated glomerulonephritis (AAGN) is a group of disorders mediated by antineutrophil cytoplasmic antibody (ANCA), pathologically characterized by pauci-immune necrotizing crescentic glomerulonephritis, its main clinical manifestation is rapidly progressive glomerulonephritis, with some patients rapidly progressing to end-stage kidney disease. Timely and accurate identification of prognostic indicators is vital for guiding clinicians- decision-making on treatment and improving patients- prognosis. The recently developed AAGN renal risk score (ARRS) incorporated the percentage of normal glomeruli, percentage of tubular atrophy and interstitial fibrosis and estimated glomerular filtration rate at diagnosis to predict kidney survival.This review focuses on the latest research progress of the ARRS model.
