Effects of donor hypothermia on renal function recovery in renal transplant recipients

摘要: 目的:探讨供者亚低温对肾移植受者肾功能恢复的影响。
方法:回顾性分析首都医科大学附属北京朝阳医院2017年7月至2019年1月完成的公民逝世后器官捐献案例,并对移植受者肾功能恢复情况进行分析。按供者是否采用亚低温治疗分为亚低温组和非亚低温组,比较两组受者术后近期移植肾功能恢复情况。
结果:亚低温组入组供者26例、受者49例,非亚低温组入组供者24例、受者45例,两组供者年龄、围手术期尿量和ICU住院时间之间比较,均无统计学意义。亚低温组和非亚低温组受者年龄和术前肌酐之间比较,无统计学意义。两组受者术后肾功能延迟恢复(DGF)发生率分别为408%和1778%,有统计学意义(P=003)。两组受者术后均无原发性无功能(PNF)发生。亚低温组受者术后1周和1月的肌酐值低于非亚低温组,术后3月和6月基本一致,但差异均无统计学意义。
结论:供者亚低温可降低受者DGF的发生率,有利于肾移植受者的早期恢复,不影响近期肾功能恢复。

Abstract: Objective:To investigate the effect of mild hypothermia on renal function recovery in renal transplant recipients.
Methodology:The cases of organ donation after death completed in our hospital from July 2017 to January 2019 were retrospectively analyzed (age from 18~65 years),and the renal function recovery of transplant recipients was analyzed. The recipients were divided into hypothermia group and nonhypothermia group according to whether the donor was treated with mild hypothermia or not. The shortterm and mediumterm recovery of renal function after transplantation in the two groups were compared,including the incidence of delayed renal function (DGF) and primary nonfunctional function (PNF).
Results:There were 26 donors and 49 recipients in the mild hypothermia group,24 donors and 45 recipients in the nonmild hypothermia group.There was no statistical significance in the comparison of donor age,perioperative urine volume and ICU stay in the two groups (P>005).There was no significant difference in receptor age and preoperative creatinine between the two groups (P>005).The incidence of DGF in the two groups was 408% and 1778%,respectively,which was statistically significant (P=003).No PNF was found in both groups.Serum creatinine values in the mild hypothermia group at one week and one month after surgery were lower than those in the nonmild hypothermia group,and were basically the same at 3 months and 6 months after surgery,but the differences were not statistically significant (P>005).
Conclusion:
Mild hypothermia can reduce the incidence of DGF,which is beneficial to the early recovery of renal transplant recipients,and has little effect on the renal function in the short and medium term.
