长透析龄伴钙化防御患者合并糖尿病足

Calciphylaxis in long-term hemodialysis patient complicated by diabetic foot

摘要: 1 例 60 岁女性 2 型糖尿病终末期肾病患者,维持性血液透析治疗 17 年余,并发罕见且危重的钙化防御合并糖尿病足。患者因右侧第 3、4 足趾发黑、疼痛入院,合并心房颤动。入院后诊断为钙化防御 (基于典型皮损、全身血管钙化及危险因素)、糖尿病足并感染。治疗采用多学科协作模式,经过 9 个月的综合干预,患者足部溃疡完全愈合,但右足第 3、4 趾因缺血性坏死脱落。本病例提示,对于长期透析合并钙化防御及糖尿病足患者,早期硫代硫酸钠治疗联合多学科管理可有效降低截肢风险,改善预后。然而,肢体远端血运障碍仍是治疗难点,需进一步探索更优干预策略。

Abstract: This report describes a 60-year-old female patient with end stage renal disease (ESRD) who had received maintenance hemodialysis for over 17 years and developed the rare and severe complication of calciphylaxis combined with diabetic foot. She was admitted due to blackening and pain in her right third and fourth toes, with concurrent diagnoses of type 2 diabetes mellitus and atrial fibrillation. Following admission, she was diagnosed with calciphylaxis (based on characteristic skin lesions, systemic vascular calcification, and risk factors) and infected diabetic foot. Employing a multidisciplinary collaborative approach, the patient underwent 9 months of comprehensive intervention. Her foot ulcers healed completely; however, the right third and fourth toes underwent ischemic necrosis and autoamputation. This case highlights that for long-term dialysis patients with calciphylaxis and diabetic foot, early treatment with sodium thiosulfate (STS) combined with multidisciplinary management can effectively reduce amputation risk and improve prognosis. Nevertheless, distal limb ischemia remains a significant therapeutic challenge, warranting further exploration of optimized intervention strategies.
