Kidney injury in patients with COVID19 the association with outcome

摘要: 目的:回顾性分析新型冠状病毒肺炎(简称新冠肺炎)住院患者肾功能损害及其对预后的影响。方法:收集火神山医院新冠肺炎住院患者2 391例,其中普通型1 263例,重型1 044例,危重型84例,分析不同临床分型患者肾功能损害及其对预后的影响。结果:危重型和重型死亡率分别为512%(43/84)和05%(5/1 044),普通型无死亡。危重型血清肌酐、尿素氮、胱抑素C和尿酸水平明显高于重型和普通型,危重型估算的肾小球滤过率(eGFR)明显低于重型和普通型。尿蛋白阳性率31%,尿隐血阳性率125%;急性肾损伤(AKI)发生率43%,其中普通型08%,重型44%,危重型524%。 多因素回归分析显示影响患者预后的肾外危险因素包括:临床分型、高龄(>65岁)、血中性粒细胞计数升高、淋巴细胞计数降低、谷草转氨酶升高、肌酸激酶同工酶升高。校正肾外因素后多因素分析显示,肾功能损害也是预后不良的高危因素:血清肌酐升高、胱抑素升高、AKI、eGFR<60  ml/(min·173m2)。结论:新冠肺炎患者可发生AKI,病情越重AKI发生率越高。校正肾外因素后多因素分析显示血清肌酐升高、胱抑素升高、AKI、eGFR<60  ml/(min·173m2)均可影响患者预后。

Abstract: Objective:The retrospective study aims to analyze associations between renal function impairment and prognosis in hospitalized cornoa virus disease 2019(COVID19) patients.Methodology:A collection of 2 391 patients with COVID19 in Huoshenshan Hospital,including 1 263 cases of general type,1 044 cases of severe type,and 84 cases of critical type,were collected to analyze characteristic of renal dysfunction  and their impact on prognosis.Results:The mortality rate for critical,severe and general groups was 512% (43/84),05% (5/1 044) and 0 respectively.Serum creatinine,urea nitrogen,cystatin C and uric acid were significantly higher in critical groups than in other groups,eGFR in critical group was significantly lower than that in the general and severe groups. The rate of proteinuria and hematuria was 31% (58/1 831) and 125% (229/1 831) respectively.The total  incidence of AKI was 43%,of which the general group was 08%,severe group was 44% and critical group was 524%. Multiple regression analysis showed that prognostic factors included disease severity,advanced age (>65),increased blood neutrophil count,lower lymphocyte count,elevated aspartate transaminase(AST) and creatiue kinaseMB(CKMB).The multiple regression analysis adjusting for extrarenal factors showed that factors were also the risk factors of poor prognosis,such as increased blood creatinine,elevated cystatin C,AKI and eGFR<60 ml/(min·173m2).Conclusion:AKI can occur in  COVID19 patients.The incidence of AKI increased with increasing severity. Multiple regression analysis showed that after adjusting for extrarenal factors,indicators of increased blood creatinine,elevated cystatin C,AKI and eGFR<60  ml/(min·173m2) have prognostic values.
