摘要: 代谢综合征(MetS)的核心病理机制为胰岛素抵抗,其与肌肉减少症相互促进,形成恶性循环。研究表明,血清肌酐与胱抑素 C 比值(CCR)可综合反映肌肉代谢与肾功能状态,在肥胖、糖尿病、高血压等 MetS 核心组分中展现出独特的预测潜力。此外,CCR 与代谢相关性脂肪肝病、冠心病等并发症的关联研究提示其作为动态监测指标的临床意义。然而,当前研究存在性别差异显著、结果不一致等局限性,未来需通过大规模流行病学调查与临床研究进一步阐明 CCR 在 MetS 预后评估及个性化干预中的作用,为降低 MetS 相关疾病负担提供科学依据。本文对 CCR 在 MetS 及其常见并发症中的研究进展做一综述,旨在系统总结 CCR 作为肌肉质量与肾功能评估指标在 MetS 早期诊断、风险分层及疾病管理中的价值,并为完善 MetS 诊疗标准提供新思路。

Abstract: This review focuses on the research progress of creatinine to cystatin C ratio (CCR) in metabolic syndrome (MetS) and its common complications, aiming to systematically summarize the value of CCR as an indicator for assessing muscle mass and renal function in the early diagnosis, risk stratification, and disease management of MetS, while providing new insights for improving its diagnostic and therapeutic standards. The article highlights that insulin resistance, the core pathological mechanism of MetS, interacts with sarcopenia, forming a vicious cycle. Studies demonstrate that CCR comprehensively reflects muscle metabolism and renal function status, exhibiting unique predictive potential in key MetS components such as obesity, diabetes, and hypertension. Additionally, research on the association between CCR and complications like metabolic associated fatty liver disease (MAFLD) and coronary heart disease underscores its clinical significance as a dynamic monitoring marker. However, current studies face limitations including significant gender disparities and inconsistent results. Future large⁃scale epidemiological investigations and clinical research are required to further elucidate the role of CCR in prognosis evaluation and personalized intervention for MetS, thereby providing scientific evidence to reduce the burden of metabolic syndrome⁃related diseases.
