摘要: 肠道菌群作为 “隐形代谢器官”,过去十年间经历了从 “新大陆” 探索到 “新范式” 建立的革命性转变。肠道菌群通过微生物⁃宿主互作调控能量平衡、免疫稳态及内分泌信号,其生态失调与肥胖、2 型糖尿病、代谢相关脂肪性肝病(MASLD)等代谢性疾病密切相关。本文系统阐述了肠道菌群在代谢性疾病中的机制研究进展、靶向干预策略及临床转化挑战等医学 “新范式”,为代谢性疾病防控提供新视角。

Abstract: Over the past decade, the gut microbiota (GM), often known as the "shadow metabolic organ", has undergone a revolutionary metamorphosis from the exploration of a "New Frontier" to the establishment of a "New Paradigm". This complex microbial ecosystem regulates critical host functions, energy balance, immune homeostasis and endocrine signaling through dynamic microbiome⁃host crosstalk. GM dysbiosis is now firmly implicated in the pathogenesis of metabolic disease, including obesity, type 2 diabetes and metabolic dysfunction⁃associated steatotic liver disease (MASLD). In this Review, we discuss state of the art for gut microbiote in metabolic disease. We introduce the evolving mechanistic understanding of how GM may link to the pathogenesis of common metabolic diseases, and explore the microbiota⁃targeted interventions to optimize metabolic health, which provides perspectives for future basic and translational investigations within the metabolic diseases field.
