摘要: 快速腹膜溶质转运 (PSTR) 是腹膜功能障碍常见表现形式,可继发腹膜超滤衰竭,增加腹膜透析 (PD) 患者死亡风险。对于快速 PSTR 患者,当前指南推荐缩短葡萄糖 PD 液留腹时间、选用更高浓度的葡萄糖透析液,或使用艾考糊精透析液。根据渗透剂种类,PD 液可分为晶体型 (如葡萄糖、氨基酸) 与胶体型 (如艾考糊精),两类液体跨膜作用机制不同,其组合运用有望发挥协同效应,改善快速 PSTR 腹膜超滤下降问题。

Abstract: Fast peritoneal solute transfer rate (PSTR) is a common manifestation of peritoneal dysfunction, which can lead to peritoneal ultrafiltration failure and increase the mortality risk for patients undergoing peritoneal dialysis (PD). For patients with fast PSTR, current guidelines recommend shortening the dwell time of glucose PD solution, selecting a higher concentration of glucose PD solution, or using icodextrin PD solution. PD solutions can be classified based on the type of osmotic agent into crystalloid PD solutions or colloid PD solutions, with glucose PD solution and amino acid PD solution representing the former, and icodextrin PD solution representing the latter. The transmembrane action mechanisms of crystalloid and colloid PD solutions differ, and their combined use is expected to exert a synergistic effect, thereby improving the issue of decreased peritoneal ultrafiltration associated with fast PSTR.
