白藜芦醇保护碘对比剂相关的急性肾损伤的实验研究

摘要: 目的:探讨白藜芦醇对碘对比剂相关的急性肾损伤的保护作用。
方法:24只实验兔随机分为四组:对照组(Control)、白藜芦醇组(Res)、对比剂相关的急性肾损伤组(CAAKI)和白藜芦醇治疗组(CAAKI+Res,每组6只)。所有实验兔在造模成功3 d时行血氧水平依赖磁共振成像(BOLD),测量肾脏皮质(CO)和髓质(OM)表观横向弛豫速率(R2*)值,观察肾脏病理改变,检测沉默接合型信息调节因子2同源蛋白1(SIRT1)、缺氧诱导因子1α(HIF1α)及其他靶蛋白表达。
结果:与CAAKI组相比,CAAKI+Res组的肾CO(P=0015)、OM(P=0002)R2*值下降,肾脏病理损伤减轻(P<0001),胶原纤维沉积减少(P<0001),SIRT1蛋白(P<0001)、B细胞淋巴瘤(Bcl2)蛋白(P=0014)升高,HIF1α蛋白(P<0001),活化半胱氨酸天冬氨酸蛋白酶3蛋白(P=0018)、Bcl2相关X蛋白(Bax)蛋白(P=0004)、CytC蛋白(P=0033)表达降低。
结论:白藜芦醇可能通过抗缺氧、抗纤维化、抗肾小管凋亡发挥在CAAKI中的保护作用,而磁共振BOLD技术可用于评价白藜芦醇对CAAKI的保护作用。

Abstract: Objective:To study the protective effect of resveratrol on contrastassociated acute kidney injury.
Methodology:Twentyfour rabbits were divided into 4 groups (n=6): Control group (Control); Resveratrol group (Res); Contrastassociated acute kidney injury group (CAAKI); Resveratrol treatment group (CAAKI+Res). All rabbits underwent blood oxygen level dependent magnetic resonance imaging, R2* values of renal cortex (CO) and medulla (OM) were measured. Renal pathological changes, cleaved caspase3 and silent information regulator 2 homologous protein 1 (SIRT1), hypoxia inducible factor1α (HIF1α) and its downstream signaling pathways were estimated.
Results:Compared with CAAKI group, R2* in OM (P<0001) and CO (P=0003) decreased significantly in CAAKI+Res group. Pathological injury scores showed that renal injury in CAAKI+Res group was less than that in CAAKI group (P<0001). Compared with CAAKI group, SIRT1 (P<0001), Bcl2 protein (P=0014) increased and HIF1α (P<0001), cleaved caspase3 (P=0018), Bax (P=0004) and CytC (P=0033) decreased in CAAKI+Res group.
Conclusion:Resveratrol plays a protective role in CAAKI mainly through antihypoxia, antifibrosis and antitubular apoptosis. Blood oxygen level dependent magnetic resonance imaging technique can evaluate the protective effect of resveratrol on CAAKI.
