摘要：
中年男性患者，临床表现大量蛋白尿、镜下血尿，伴低白蛋白血症、急性肾损伤、高血压、贫血等。血免疫固定电泳示κ型Ig G单克隆免疫球蛋白条带。肾活检病理示肾小球膜增生性病变伴κ轻链沉积，电镜下观察肾小球毛细血管袢内皮下大量微管状结构，直径14～22 nm，最终诊断为伴微管状κ轻链沉积的增生性肾小球肾炎。

Abstract:
A 62yearold male presented with nephrotic syndrome,microhematuria,renal insufficiency,anemia and hypertension.Serum immunofixation electrophoresis assay revealed IgG κ monoclonal band.Renal biopsy demonstrated membranoproliferative glomerulonephritis with monoclonal immunoglobulin deposits of kappa light chain.On electronmicroscopy,there were a lot of microtubules in subendothelial region,ranging in size from 14 to 22 nm.Finally,the patient was diagnosed with proliferative glomerulonephritis with microtubular deposits of monoclonal kappa light chain.
