Abstract: A 67-year-old male, presented with serum creatinine increased gradually accompanied with few proteinuria. The patient underwent Whipple surgery for pancreatic tumor 3 years ago and took Chinese herbal tea in recent months. Renal biopsy revealed interstitial and tubular lesions (acute on chronic), a large amount of crystal deposited in renal tubular lumen which exhibit colorful refraction under polarized light, ultimately diagnosed as oxalate nephropathy due to secondary hyperoxaluria. The renal function improved after 3 months of treatment with a low oxalate diet, plenty of drinking water, vitamin B6 and cholestastyramine.
