Abstract: Recently, with the indepth study of preoperative pretreatment and prevention of postoperative complications, ABO incompatible kidney transplantation (ABOiKT) has become an important way to alleviate kidney shortage. However, patients with ABOiKT still face problems such as infection, bleeding, and antibodymediated rejection, which affect the longterm prognosis of recipients/transplanted kidneys. Current studies on ABOiKT mainly focus on the exploration of more effective preoperative management, reasonable prevention and treatment of postoperative complications. In this paper, based on the existing preoperative management scheme and the prevention and treatment of postoperative complications of ABOiKT as the entry point, combined with the latest research, the future research direction of ABOiKT is discussed, so as to provide reference for further improving the longterm survival rate of kidney transplant recipients and graft.
