摘要老年女性患者，临床表现为类风湿性关节炎，未服用金制剂、青霉胺等药物，尿液检查发现中至大量蛋白尿伴少量镜下血尿，肾活检组织学为肾小球膜性病变，伴有新月体和袢坏死，实验室检查发现抗中性粒细胞胞质抗体阳性，结合临床。考虑类风湿性关节炎肾损害。

Abstract:

ABSTRACT A 59-year-old woman with long-standing rheumatoid arthritis (RA) developed proteinuria and hematuria. Neither gold nor penicillamine had been administrated. Renal biopsy showed membranous nephropathy with capillary necrosis and crescent formation. Laboratory examinations revealed antineutrophil cytoplasmic antibody positive. The final diagnosis was rheumatoid arthritis associated with nephropathy.
