C3aR——检测肾组织肥大细胞的敏感标志

C3aR: a sensitive marker for renal mast cell

摘要:

目的:了解C3aR高表达细胞在正常个体和各种肾脏病患者肾组织中的分布情况,对其细胞类型进行鉴定,并对其能否作为肥大细胞特异性标记分子进行评估。 方法:利用免疫组化的方法分析C3aR高表达细胞在正常供肾组织和各种肾脏病患者(包括2型糖尿病肾病、IgA肾病、急性间质性肾炎、慢性间质性肾炎、ANCA相关性血管炎、微小病变、膜性肾病、FSGS以及狼疮性肾炎)肾组织中的分布情况;利用光镜、免疫电镜、免疫荧光双套色染色和甲苯胺蓝特殊染色等方法对肾组织中高表达C3aR的细胞类型进行鉴定;在此基础上,对C3aR作为肾组织肥大细胞特异性标记分子进行评估。 结果:1、C3aR高表达细胞在正常供肾组织和包括2型糖尿病肾病、IgA肾病、急性间质性肾炎、慢性间质性肾炎、ANCA相关性血管炎、微小病变、膜性肾病、FSGS以及狼疮性肾炎在内的各种肾脏病患者肾组织中均存在。2、基于光镜和免疫电镜的形态学分析显示肾组织中高表达C3aR的浸润细胞在形态上具有肥大细胞的特点;基于免疫荧光双套色染色的分析显示,这种细胞CD45阴性、CD68和类胰蛋白阳性,与肥大细胞的情况一致;甲苯胺蓝特殊染色进一步证实这是一种肥大细胞。3、虽然C3aR也同时表达于小管上皮细胞等其他细胞,但其在肥大细胞中的表达水平远远高于其他细胞,这种表达水平差异足以将肥大细胞与其它细胞区分开来。4、与目前最常用的肥大细胞标记分子——肥大细胞类胰蛋白酶相比,C3aR作为肥大细胞的标记分子具有相似的检测灵敏度和更好的特异性。结论:肾组织中的肥大细胞特异性地高表达C3aR, C3aR是肾组织肥大细胞的一个敏感标记分子,完全可以C3aR代替类胰蛋白酶,以免疫组化的方法检测肾组织中的肥大细胞。

Abstract:

Objective: To realize the distribution of C3aR highly expressing cells we found previously in the renal tissue of both normal people and patients with various kidney disease, identify the cells and evaluate the feasibility of using C3aR as a marker in detecting renal mast cells. Methodology: Renal sections of biopsies from both normal donor and patients with kidney disease, including diabetic nephropathy, IgAN, acute interstitial nephritis, chronic interstitial nephritis, ANCA associated vasculitis, minimal change, membranous nephropathy, FSGS and lupus nephritis were analyzed by histological, immunohistochemical, double immunoflurorescence, immunoelectronicmicroscopic methods and toluidine blue staining. The number of C3aR highly positive cells in each section was counted, the density of the cell was calculated, and the comparison between C3aR and tryptase as a marker of renal mast cells were made. Results: C3aR highly expressing cells were found in all renal sections from both normal donor and patients with various kidney diseases, though the number of the cell is different in different sections. The C3aR highly expressing cells, which had a morphous of mast cells, were positive in CD68 and tryptase immunohistochemical staining and toluidine blue staining while negative in CD45 staining, and thus were proved to be mast cells. Comparison between C3aR and tryptase as a marker of renal mast cells revealed that C3aR is better than tryptase as a marker in detection of mast cells in renal tissue. Conclusion: C3aR is specifically highly expressed in renal mast cells and can be used as a good marker in detection of renal mast cells immunohistochemically.
