摘要: 潜能未定克隆性造血( CHIP) 是一种与年龄相关、致病潜能不确定的造血干细胞扩增。  流行病学研究表明,CHIP 与血液系统疾病风险增加相关,且与肾脏功能下降、急性肾损伤、慢性肾脏病( 包括糖尿病肾病) 等肾脏病密切相关。  CHIP 相关基因突变( 如 TET2、JAK2、ASXL1 等) 可激活炎症相关信号通路[ 如 NOD 样受体热蛋白结构域相关蛋白 3NLRP3) 炎症小体、黑色素瘤缺失因子 2AIM2) 等],诱发局部炎症反应,促进肾脏纤维化。  本文拟从 CHIP 的流行病学、检测方法、管理策略及未来展望等方面展开论述,着重探讨  CHIP 与肾脏病的关系及研究进展,为肾脏病的预防与治疗、抗炎药物的开发与应用提供理论依据。

Abstract: Clonal hematopoiesis of indeterminate potential ( CHIP)  is a condition characterized by the expansion of hematopoietic stem cells with uncertain pathogenic potentialwhich is associated with aging. Epidemiological studies have indicated that CHIP  is  linked  to  an  increased  risk  of  hematological  disorders  and  is  closely  related  to  kidney  diseasesincluding a  decline  in  kidney  function,  acute  kidney  injury  AKI ),  chronic  kidney  disease  CKD ),  and  diabetic nephropathy. CHIP⁃related mutations, such as TET2JAK2ASXL1etc.can activate inflammation⁃associated signaling pathways [ such  as  the  NOD⁃like  recepror  pyrin  domain⁃containing  3  ( NLRP3)  inflammasomes,  absent  in  melanoma  2 AIM2), etc.],  induce  local  inflammatory  responses,  and  promote  kidney  fibrosis.  This  article  aims  to  discuss  various aspects  of  CHIP  including  its  epidemiology,  the  relationship  with  kidney  diseases,  detection  methods,  management strategiesand future  prospects,  with  a  focus  on  exploring  the  relationship  and  research  progress  of  CHIP  with  kidney diseases. The  objective  is  to  provide  a  theoretical  basis  for  the  prevention  and  treatment  of  kidney  diseases  and  the development and application of anti⁃inflammatory drugs.
