Abstract: Secondary hyperparathyroidism (SHPT) is a common complication in patients with end-stage kidney disease (ESKD). The main characteristic is elevated parathyroid hormone (PTH). Although kidney transplantation is the most effective replacement therapy for ESKD patients, the level of PTH in renal transplantation candidates not only affects the transplanted kidney itself, but also is associated with related complications. However, the treatment method and timing of SHPT before renal transplantation are still controversial. In this paper, we will review the influence of PTH level before kidney transplantation on kidney transplantation, ideal PTH level of kidney transplant recipients and timing of parathyroidectomy, to provide reference for clinical significance.
