Eveluation of cardiovascular events using  galectin3 and fibroblast growth factor23   in chronic kidney disease patients

Abstract:

Objective:To investigate the relationship between serum Galectin3(Gal3),fibroblast growth factor23(FGF23) levels with the left ventricular geometry remodeling in chronic kidney disease (CKD) patients.
Methodology:This was a crosssectional study. Patients aged 16~65 years with baseline CKD stages 2 to 5 were enrolled in this study. Baseline clinical characteristics such as gender,age,smoking,diabetes,blood pressure,body surface area(BSA),body mass index(BMI) were obtained. Serum uric acid,creatinine,albumin and Creactive protein were measured by standard laboratory techniques. Glomerular filtration rate (GFR) was calculated using the CKDEPI formula. Serum Gal3,FGF23 levels were measured by using enzyme linked immunosorbent assay(ELISA). All patients were examined by transthoracic echocardiography to measure the diameter of the left ventricle. Left ventriclar mass index(LVMI) and relative wall thickness(RWT) were obtained by calculation. The relation between NTproBNP,Gal3 and FGF23 levels with left ventricular geometry remodeling was evaluated by Logistic regression and receiver operating characteristic(ROC) curve.
Results:A total of 270 CKD patients were enrolled in the final analysis. Serum Gal3 and FGF23 concentrations with eGFR levels were strongly correlated. Other variables with moderate to strong correlations with Gal3 or FGF23 concentration in participants were diabetes,systolic blood pressure,levels of hemoglobin,albumin,phosphorus,PTH,NTproBNP,TnT and 24h proteinuria. Gal3 or FGF23 levels correlated moderately with LVM和LVMI. Areas under the ROC (95% confidence interval) were 0680 (0598,0761),0661(0565,0757),and 0641(0554,0729) for NTproBNP,Gal3,and FGF23,respectively,with no significant difference between the values (P>005). AUC was increased to 0696 for combinedbiomarker of NTproBNP,Gal3 and FGF23.
Conclusion:
Higher levels of Gal3 and FGF23 were associated with several cardiovascular risk factors and left ventricular geometry remodeling in CKD patients. Emerging cardiac biomarkers with different pathophysiological background incorporated may offer important prognostic information in the setting of CKD.
