Fractionated plasma separation and adsorption-continuous veno-venous hemofiltration in sepsis and multiple organ dysfunction after trauma

Abstract: A 52-year-old male suffered severe trauma from high-altitude falling.The patient developed multiple organ dysfunction (acute liver failure, acute kidney injury and trauma-induced coagulopathy) and sepsis. Continuous renal replacement therapy, fractionated plasma separation and adsorption-continuous veno-venous hemofiltration (FPSA-CVVH) combined with Mediasorb adsorption column enhanced cytokines clearance were administered; total bilirubin reduction ratios were 57.2%~72.3%.Moreover, high mobility group protein-1 reduction ratios were 69.0%,interleukin (IL)-1β、IL-6、IL-8、IL-10、tumor necrosis factor-α、interferen inducible protein-10、monocyte chemotactic protein-1、macrophage inflammatory protein-1β reduction ratios were more than 90%.Sepsis was effectively controlled, and liver function of the patient was alleviated quickly.The patient gradually got rid of renal replacement therapy.
