Abstract:
Secondary hyperparathyroidism (SHPT) is one of the common complications of chronic kidney disease (CKD). Circulating parathyroid hormone (PTH) was elevated secondary to the decline of glomerular filtration rate. Abnormal secretion and metabolism of PTH, along with endorgan hyporesponsiveness to PTH in CKD played a critical role in the development of SHPT. The relative mechanism had been further clarified recently. However, there are controversies about PTH as a prognostic biomarker in CKD due to its metabolic and functional abnormity. This article will summarize recent studies about abnormal metabolism and function of PTH and the potential mechanism in CKD patients.
