Abstract:
A 66yearold male patient presented with acute renal insufficiency. He had elevated IgG4 levels with monoclonal immunoglobulin IgAλ. A renal biopsy showed severe tubulointerstitial nephritis (TIN) with extensive infiltration of IgG4positive plasma cells, suggesting a diagnosis of IgG4related tubulointerstitial nephritis(IgG4related TIN) with monoclonal gammopathy of undetermined significance (MGUS).
